I just pulled this together for the folks I work with, obviously if you have an account with any of these airlines, it's smart to go ahead and sign up now, even if you have nothing planned (I've always been a bit of an airline tramp, so I fly and accumulate miles in all these programs myself!):

If I missed any from smaller airlines, please let me know!
This was great. Could we add the bonus EQMs from partner offers to the original post and make it a definitive post or sticky for all EQMs?

e.g.,

10k EQM for spending 25k on Business Juniper MC
10k EQM for spending 25k of B of A Visa

UA

35k spent in a calendar year: 5k EQM
up to $5k spent on UA.com: up to 5k EQM (1EQM:1$)
(edited to include germansoks corrections)
Buy EQMs (10 RDMs per 1 EQM)

Amex
5k MQM for opening Platinum Delta
10k MQM for spending 25k with Platinum Delta
20k MQM for spending 50k with Platinum Delta
(nontransferable)

10k MQM for opening Delta Reserve
15k MQM for spending 30k with Delta Reserve

It is also possible to earn EQM with the United Visa Platinum card (annual fee $140). Not sure if other airline affinity cards offer EQM.

Looks like ffI has a good list above. I would add that first time United Visa Platinum CC users will also receive 1 EQM for each $ charged to United.com.

You are right The first year is 15k, next yr is 10k max.
See below

35k spent in a calendar year: 5k EQM
up to $5k spent on UA.com: up to 5k EQM (1EQM:1$)

the fee for the card is $140 and not $145.
Earning EQM: Upon enrollment in the program, you will receive a one-time bonus of 5,000 EQM. Also, you can earn up to 5,000 additional EQM each Year at a rate of 1 EQM per $1 of eligible net purchases made on United.com. ("Year" means, for your first year as a cardmember, the period beginning with your Enrollment Date through your December statement date of that same year. For each year thereafter, "Year" means the twelve month period beginning the day after your December statement date through your December statement date of the next year.) As a one-time bonus each Year, once you reach $35,000 or more in Net Purchases, and you make one additional purchase within that Year, you will receive 5,000 EQM. You do not earn miles on balance transfers, cash advances, cash-like charges such as travelers checks, foreign currency, and money orders, any checks that are used to access your account, overdraft advances, interest, unauthorized or fraudulent charges, or fees of any kind, including fees for products that protect or insure the balances of your account. There is no maximum number of miles that you can accumulate in the program. If you have more than one United Visa account you can earn EQM on each account. EQM will appear on each account statement; however, under the United Mileage Plus Platinum Class terms and conditions, the maximum number of EQM that can be awarded to your United Mileage Plus Member Number is 15,000 the first calendar year and 10,000 each calendar year thereafter. Any excess EQM will be forfeited. EQM are not redeemable for travel at United. EQM are used solely toward flight status upgrades of Elite Status. See Rewards Program Rules and Regulations, which will be mailed after your account is established.

Does anyone know of any other Tier Miles Bonus Programs being offered by any of the airlines now, or in the not too distant future, especially Delta or Northwest?

AS DEQM is good through June 30 (though the better transcon fares are long gone- at one point, a DCA-LAX-SEA-PDX-ANC r/t could net you some serious mileage for fairly cheap, when adding in an elite RDM bonus). I have had luck getting flights credited on AS metal even when the ticketing carrier isn't AS (in my case, AA), so it's possible you could find a lower fare on AA, NW, DL or CO if you looked. I doubt that any flight not on AS/QX metal will credit DEQM, though.

AS status does provide domestic upgrades/elite seat selection on NW (and supposedly will carry over to DL in 2010- it's probably equivalent to DL Silver Medallion, without the Skyteam elite status), as well as on AS, and you can earn full AS EQM on AS/QX, AA, NW, DL, KL, AF, and LA, as well as RDM on BA, QF, CX and CO (and some other smaller airlines).

AS is also pretty nice about status matches, FWIW.

AA Terms and Conditions
I needed to dredge this up (I booked a trip on 3/16, and flew it on 3/25, and it didn't qualify) so I thought I would post the Terms and Contions here, since they won't last forever on aa.com

Airline: American Airlines
Award: Double Elite-Status Qualifying Miles
Bonus Code: DBEQM

Dates: Register today by selecting the red "REGISTER" button below. Book your trip and travel between March 18, 2009 and June 15, 2009.

Offer: Qualify or re-qualify for AAdvantage Gold, AAdvantage Platinum, or AAdvantage Executive Platinum status faster than ever with this special promotion. Just book your trip and fly on American Airlines, American Eagle, or AmericanConnection between March 18, 2009, and June 15, 2009, and you'll earn double elite-qualifying miles - the miles that bring you closer to AAdvantage elite-status membership - at an accelerated rate.

Register prior to travel to qualify for this special offer by selecting the red "REGISTER" button below.

Terms and Conditions:
Double elite-status qualifying miles are not redeemable for award travel, but do count toward qualification for AAdvantage Gold, AAdvantage Platinum, and AAdvantage Executive Platinum status. AAdvantage elite-status qualifying double-mile offer is valid only for travel on eligible purchased, published fare tickets on American Airlines, American Eagle, or AmericanConnection marketed and operated flights between March 18, 2009, and June 15, 2009. Promotion does not apply to travel on mileage award tickets or fares that are ineligible for AAdvantage mileage accrual. Flights operated by our codeshare partners or oneworld alliance carriers are not eligible for this promotion. Offer applies only to new reservations booked between March 18, 2009, and June 15, 2009. Registration prior to travel is required. This offer does not increment the member's elite-qualifying points, segments or award-eligible mileage balance. Double elite-qualifying miles will be calculated at 100% of the elite-qualifying miles earned for each applicable flight and will be posted to the account of the traveling AAdvantage member within six to eight weeks after the qualifying activity is posted to your account.
