Summary of FF programs charging fuel surcharge?

Is there a summary somewhere of which frequent flier programs charge fuel surcharges on their own flights and/or partner flights?

For example, I just became aware that BA Executive Club and CX Asia Miles charge fuel surcharges on award flights booked on BA, whereas AAdvantage does not.

I came across a handy roundup on SmarterTravel recently - they've got a PDF listing twelve of the common frequent flyer redemption fees for US-based airlines.

Fuel surcharges are perfectly in character for an anti-consumer ff program like DL SkyPiles and its fearless leader, Jeff Robertson, who is picking up where Rob Borden left off in screwing customers. It is sad for NW, which is undoubtedly following along like a pathetic puppy dog following DL, knowing that in a few months it will cease to exist, being engulfed by the DL deathstar. This is really out of character for the old NW which has stood up for customers and refused to go along with some of DL's anti-customer crap in the past. NW standing up for customers and refusing to copy Rob Borden's crap is probably the main thing that stopped it from spreading to other airlines and eventually forced DL itself to reverse course.
