Has any defunct carrier been successfully restarted?
I remember that Branniff and Pan Am have both tried recussitation, not too successfully.
Have any carriers gone under (not just emerging from bankrupcy), but REALLY under, and then been restarted successfully?

Seems like great fodder for TravelBuzz since it isn't miles and points related (except insofar as a surviving carrier might or might not have honored the accumulated mileage balances of its predecessor FFs... 
).
