flew in early Sept and did not know of this one but thanks. Will try it for next trip. If it still works, I am thinking one more little thing may help all who fax the form in:
Reduce it when printing it out so the text does not end up going to the end of your page. Consider that you are faxing to the land of A-4 sized paper and not all machines work the same.
