I have gone negative with American by around 10 to 15k miles when I needed an award right away (i.e., for travel the next day). I think they charge a fee, but it seemed reasonable at the time.

Negative with DC after transferring points and then cancelled some trips in the fall.

Also, was momentarily negative with BA when I got a rewardwhich was a couple of hundred more miles than I had. Agent put the miles in for free to zero me out.

Last August I went -74,963 points with HH. It was a legit negative but I did promise to send back send back a 120,000 point award while they agreed to send the new one out right away.

I was -120K or so on AA this past summer. It lasted a couple of days. They were moving 1mm+ over from TWA and I wanted to claim 2F to Europe so I was temporarily in the red. Apparantly moving over that many miles required some kind of special 'audit.'

My wife went negative with HHonors 3 years ago due to an award mixup. It took me 2 years to straighten out. In the end, HH put a 80K bonus into our now joint account for their repepated screwups!

Went negative (about 20'000) for a while with Swissair's Qualiflyer programme as a Travel Club member. That lasted about 6 months.
