Essentially, they are first status sorted (1K, 1P, 2P) and then fare sorted. The rules are somewhat different re: advance requests and airport standby. There are myriads of discussions on the UA section of this Board, as well as an "unofficial UA" page which is very helpful. Just scan through the UA messages and you will learn more than you ever wanted to, very quickly.

The upgrade lists (called DM or departure management lists)break down as follows.
when one checks in and requests an upgrade one is added to the DM list. (they usually give you a card that says you're on it)
1ks go to the top of the list immediately, followed by p2 and p1 (premier exec and premier)
among the different groups the list is broken down by mileage. whoever flew the most miles last year is first. If two 1ks have the same miles (it happens) the the one who has been a mileage plus member longer goes first.

So even the time you check in may have no bearing since someone (same MP status)who has a higher fare basis could check in later than you but move ahead of you on the waitlist.

For international travel YTD mileage seems to count. Before I was a 1K I was once told a 1K with XXX miles YTD was going to get the last seat up front.
In the AEP business class lounge the upgrade list shows mileage ytd for all people on the list.
When you have a bunch of full fare business class 1k passengers (all paying the same fare) all wanting seats up front I can't imagine any other way to do it.
