This could be a completely wrong perception, but here it goes....
There is a cohort for whom "mere" miles and status don't mean that much. They would want special services beyond just free trips.
I think that most elite flyers though are garden-variety road warriors. The super-elite I referred to in the previous paragraph are probably flying private, or can afford paid J/F, or are able to make their subordinates do the travel while they get to put their kids to bed every night.
The concierge, personalized promotions, travel assistance, those are nice for people with more money than time, or more money than they know what to do with. The ones who are flying 100, 200, 300K BIS miles a year, I think they'd just like to know that when they get all their miles, they can actually do something with them without needing a Ph.D. and a huge amount of luck, and that they can count on getting upgraded if there's a seat, and getting a more convenient flight if there's an available seat on one.
