Spreadsheet. With all of my accounts/numbers/promo information etc. Kept with an ambiguous title. I'm beyond the age when I can keep up with all of that information. Especially since some accounts which require a pin now require that the pin have six numbers instead of four!

Get eWallet or a similar program that runs on your iPhone, iPod Touch, PC, ... and keep all your card numbers, PINs, passwords, phone numbers, etc at your fingertips yet encrypted.
Get a business card organizer to hold all your physical cards that you don't need to cart around or keep them in a shoe box with rubber bands holding them together.

I went to Office Depot or a similar store and got a little black book, made to hold business cards (3 per page, in little vynal pockets), I fill it up with frequest flyer cards, hotel cards, casino frequest player cards and a couple grocery store cards (like the Kroger discount card ... there are no Krogers in FL, but when I travel I use it alot to save on beer and soda) .....
Then I keep it in my caryon when I travel. Great system. Keeps me organized. Then I will put things like bump vouchers or boarding passes loose in the book, so I know to reach for it whenever I need anything travel.

It's excellent, what I suggest you do is to go to your local department store. Here in the UK I have seen some nice travel wallets in John Lewis.

I keep the current cards in a travel wallet in my bag, and I toss the old ones in a filing cabinet. All the numbers are stored in Award Wallet, Fidelity's FullView, GoMiles, and PasswordSafe.

I have an app on my Android phone (Sony Ericsson X10 Mini) called MySafe that allows me to store all card/program/account info under one single secure code.
