The only remaining outbound seems to be October 4. After trying the whole day to get any return at the low K fare, and just before giving up, something drove me to try breaking the return in Bogota. That worked for several dates. I'm not enthusiastic about an overnight in that country, but the price is right. Oh, and this includes the domestic round-trip to Cusco!
So, if anyone else wants this, note that for the LIM-BOG leg, Expedia produced a late morning flight through Quito, which doesn't work for this deal. You have to enter the specific 6:00am hour for this leg to make Expedia pick the Avianca non-stop to BOG.

Expedia won't show me the fare class (or I'm not looking where I should), but I believe that the TACA flights LIM-CUZ-LIM will earn miles on UA, if they are in the following classes: Y B E G H I K L M N O Q R S U V W Z
For the Avianca flights, I will of course enroll in AviancaPlus and I think that I will get miles even for K.

Selecting 6am on leg 4 made Expedia pick the flight I wanted. The default of "Anytime" brought up a different flight. It was just intuition that made me try that; I'm certainly not some expert...
If you're trying this and failing - so am I now. I must have grabbed the last two seats, but keep trying with different return dates and include 1-2 days stopover in Bogota on the return. Good luck!

I am joining Avianca's program and will look into partners for the TACA flights.
I have learned long ago that the best way to be able to maximize my time and resources for MR's and these bargain-basement fares is either to subscribe to something like expert flyer or have a good TA. As has been posted here many times, trying to find limited seats without a tool that shows the fare buckets is next to impossible.

I just found a $224 fare into CUZ, but it would have allowed for only 24 hours on the ground there--not enough time to visit Machu Picchu.
My point in all my recent posts is to show that the fare is still available even this afternoon--it just takes some searching.
