A cab to downtown hotels is about $20.00. A list of some of the average fares is here http://taxi.state.nv.us/FaresFees.htm
Other options would be to use one of the shuttle companies for about $6.00.

Somewhere here there's a lively thread about cab drivers being prone to take a long looping route on a freeway through some tunnel rather than the direct route through town.

CAT Bus to Downtown: $1.25
In the believe it or not category: I was waiting for the Citizens Area Transit (CAT) bus from the airport to the Downtown Transportation Center one day, when I noticed an Asian couple running to board. This couple looked to be in their sixties, but were wearing LA-style gangsta threads. How interesting, I thought! After some time, I chatted them up. It turns out they were Japanese tourists, from Tokyo. I told them I stayed at a cheap, Shitamachi (downtown) hotel when I visited Tokyo. They told me I was staying in "the slums". (Irony not lost) Anyway...they love gambling, they love Las Vegas and they apparently don't like to part ways with their money in extraneous manners. So, they bus it to downtown and do their thing at El Cortez!, where they're probably comped rooms.
The regular fare is $1.25. I don't find the ride scary or intimidating, however, there will be smelly people and quite possibly crazy people aboard. I believe there are two routes from the airport, and they are both pretty straight shots. Just take the elevators to the Zero Level of the parking garage and walk angle right when you exit to outside. CAT bus website.

The CAT 108 only goes north from McCarren, the 109 goes both north and south; since downtown is north from the airport, make sure you take 109 in the right direction.
(As noted these can both be boarded on the ZERO level.)
The CAT can actually get you downtown faster than the shuttle. The shuttles, usually, must first fill-up before departing--this could take 30 minutes; then they drop off passengers at their hotels. (For me, I seem to be the last one being dropped off, no matter what hotel I am staying at

We had that happen, however, as New Yorkers we let him know pretty directly that we knew what he was doing and that we didn't appreciate it. Needless to say the meter was turned off. Grrrr. It definitely made me sympathize with the tourists that come to NY as it's not as easy to tell if a NY cab driver is taking advantage of you from an airport!

The tunnel is not always a bad choice. You would certainly want the cabbie to use the tunnel to get downtown. The tunnel is the easiest route to the freeways. Depending on day of the week and time of day the tunnel can be a time saver.

kaukau describes a cleaner version of CAT Route 109: up Swenson. I would take roughly CAT Route 108: up Maryland Parkway. Either route the traffic will move smoothly and in a fairly direct manner to downtown. The tunnel first takes you south (away from downtown) and then dumps you on the 215, where you can then head north on the 15. I've always seen back ups going from the 215 to the 15, and I've always seen snarls on the 15. Those have been my experiences. YMMV
