Just did this a few days ago, it was quick and easy. Passengers connecting to other flights go through a security screening, basically they check your boarding pass and run you through x-ray and metal detector. Very lenient too, no need to remove your shoes or take out your liquids. For me the walk was very short from my arrival gate to the security location, but I can't say how many of these security locations for connecting passengers there may be in the airport, as it was my first time. Also, I arrived very early in the morning, so there were not many other passengers to contend with. After the security screening, you head directly up an escalator to the gate area.
Overall, I found ICN to be very efficient and an easy layout to navigate. It's a beautiful facility.

You shouldn't have much problem making your flight. Its possible you could have a very short walk to the security checkpoint or a very long one, just depends on which gate you come into. Security is pretty quick and easy once you get there, however at some times of the day the lines tend to get a bit long. I wouldn't be too concerned about making your flight but you won't likely have any time to stop and enjoy the shopping at ICN and they will likely have started boarding by the time you get there so if the flight is full overhead space may be limited.

I'm flying through ICN in March with a six hour layover (late afternoon / evening). Is it a pretty straightforward deal to clear Customs & Immigration and get into the city for a couple of hours? Any airport departure tax on the way out? Easy access via train or subway to Seoul?

Immigration has been pretty fast most times I've been there. I've only seen one time where the lines were long, but if you walked 5 minutes down the terminal to the next set of immigration stations there was no line. I don't think I've ever waited more than 3 minutes for immigration either going out or coming back in. Security on the way into the terminal on the other hand can take longer. I've done it in 5 minutes in the evening and 45 minutes during the daytime, so YMMV.
The bus downtown takes about an hour each way. Do some research and find out where you want to go, there are a ton of express bus options outside the arrivals area. They all go different places and don't expect the driver to speak any english to guide you. Actually once you leave the airport, don't expect anyone to speak much english. If you get lost most taxis have a translation service by cell phone, taxis are pretty cheap in Seoul also. (compared to the relative cost of everything else there)
The train option, should you choose to try it goes to the old Gimpo Airport. From Gimpo you have to take the subway/taxi/bus to where you want to go. In a couple of years the train will go all the way to Seoul Station, but that's not going to help you now. Getting to Gimpo on the train is fairly quick, but not cheap. Getting anywhere from there takes forever. Seoul subways have a lot of stops and are not very fast.
With your 6 hours, you'll probably get to spend about 2 hours or so downtown depending on how fast you get off your flight and through immigration.

As much as I enjoy leaving airports in favor of brief city visits, a 6-hour layover does not allow for a lot of time in Seoul. Getting to town (immigration, waiting for the bus, bus trip) may easily take two hours. For the return bus trip, you have to budget another 60-90 minutes. If you want to be back at ICN 60-90 minutes before your flight, you'll probably end up with less than 2 hours in Seoul. I once took the bus to Itaewon on a similarly short layover (to catch a quick lunch with an old friend), but ime it'd be pointless to go through this hassle and stress for some "sightseeing."

You might want to consider one of those transit tours they actually offer through the airport . (Go to the official Incheon Airport site and you will find it there) You go passed the door that spilts off the transit passengers and about 10 meters to the left is the desk for the transit tours right next to immigrations. Customs and immigrations in S. Korean is such a pleasure!!! Traffic on the other hand is a whole different story.
