No need for paper certificate (I don't think they issue them anymore, at least as far as KL metal flights are concerned). Deadline is in principle two weeks before departure.

I'm always issued paper tickets when flying KLM metal !
Anyway, if you want to upgrade online you'll need the ticket number. If you don't have it give the PE desk a ring and they'll check your booking in their system.
I've never booked an upgrade two weeks before, usually just a couple of days and then I always call before I book to make sure there's availability.
I'm not recommending this though, maybe I've just been lucky so far .

They DO send out paper certificates. I upgraded a person not related to me on a flight in Aug(?) without problems, but needed to send her the certificate.

I've been promised the paper certificate many times, but have yet to receive one in the mail.
Once a presumably inexperienced agent at an Asian KLM outpost assumed my upgrade was a computer error, as I had no paper evidence for it.
Nowadays I always go to the GVA ticket office to have a 'D' sticker affixed on flight coupons for my upgraded segments, prior to departure.
