US Attestation form

Hi everyone,

Flying BCN AMS LAX this weekend (with overnight in AMS) and it was pointed out that it is required to fill in the US attestation form.
I am fully vaccinated and have planned to take the test after the BCN-AMS segment, during my overnight stay in AMS, so it's <1day when I board the AMS-LAX segment.

I noticed VS allows this to be done online and I believe KLM doesn't have such a service/feature at the moment.

Anyone can share their experience on how this attestation form submission works for flights to the US?

Few questions I have;
- My ssumption is this form needs to be printed out and handed in - correct?
- Do I have to submit this during check-in at BCN? If not, should I carry it with me?
- If not, how would this work in AMS? During check-in or at the gate?
- Anything I have missed?

Yes, on paper (printed on both sides, not 2 pages). It will be taken at the gate in AMS or at any intermediate paperwork checkpoint they may have eventually in AMS.

Thanks for clarifying this.

About the requirement for it to be printed on both sides, that's not mentioned in any of the emails i've received from KLM nor mentioned on the CDC website. I have it already printed on single pages (at a print shop....)
Am I overlooking something here?

Yeah my thoughts as well, hence I frantically looked at all requirements but couldn't find that one

At least the last time (mid-Nov) when I flew with KLM, this was not asked or needed anywhere (I even checked at the check-in counter) for KLM operated flights. I was told this IS needed and indeed collected at the gate if the flight is operated by Delta.
I had it printed double-sided, but no idea if that is allowed or not since it ended up in the bin.

Seems to be an airline decision . All of my AF/DL flights to the US have required the paper version while UA does the attestation online.
