Website strikes again double book

Was purchasing a one way flight for my Mom with miles.
At the point you pay the taxes I kept getting an error that my credit card information was incorrect and to resubmit.

When it did go thru I only got one emailed confirmation and one credit card charge BUT... Yup they created two confirmations, one unticketed, and took the miles twice.

Four minutes on hold and about 13 minutes with the rep and I got the miles back.

I was actually checking another flight under Manage Flights when I discovered the two bookings.

I actually had a similar issue a couple months ago except mine booked but didn't ticket. I had to call as well to resolve over the phone.
