How familiar are you with NRT? Can you read Japanese? Is this the first time in Japan?
Only asking because if you are not so familiar with the layout then you should add padding to the schedule.
Approximately three hours in Harajuku is not a lot of time but if you have specific things you want to do, it would be worth it. If it is to just look around, it would feel really really rushed. You would have to do a cost benefit comparision.

I came across this the other day. A nice "official" NRT transit guide with a chapter on long layovers and stepping outside of the airport.
I wouldn't base my trip on it but it looked interesting in a glossy tourist guide kind of way.

And Ueno Park directly across from Ueno station has many major museums such as the Tokyo National Museum. Admission fees are reasonable, ~600 yen. There's also a cool shopping/dining neighborhood a few minutes' walk away from the station. Definitely an option.

I have a 5 hour layover on Wed. Worthwhile to get out? Arriving SQ638 at 0800 and departing AA176 at 1315.
Flying business class, not sure if that helps reduce time to get back to gate through expedited security lines, etc.
Likely need AA to print me a boarding pass on my return to the airport, but will ask SQ in SIN if they can since all flights are on the same booking.
