How to get miles back
I had about 50k miles on Frontier primarily from my Midwest airlines travel. I had checked a couple of months ago and I had recent rental car activity to keep my account in good standing. I tried to login to my account yesterday and had to change my password. When I did, there was no FF account associated with my account number. What happened to my miles and how do I get them back?
I tried to email them but had no luck because the only option had to include flight information, which wasn't relevant for my issue.

I have had sucess on another matter using the email comment form. Just fill in whatever additional junk information needed to get it to work. Took them two weeks to reply, about a month ago. You might also try to call, but that didn't work for me, the people I got had no idea what I was talking about. You could also try @FrontierCare on twitter.
That said did your account expire? They recently changed the expiration time to 6 months. Others have also reported miles going missing.

My miles shouldn't have expired. When I login, I can't even access a frequent flyer account. If they expired, I would think I could access my account and it would have a 0 balance. Instead, it asks me to sign up for an account. There appears to be no link for my account number to an existing FF account.
