I've left a (nearly) $1000 tip once, but it was on a nearly-$5000 pretax tab for 80-something people -- was really bloody amazed that the restaurant in question did NOT have a set gratuity for large parties, although we knew that when we booked the place and budgeted for it. Proportionally that's not so big, though.
(Looking back, wish I'd had a mileage card back then; that one was by check!)

This was booked quite a while in advance, and they seemed quite used to being paid by check for large private parties. I can't remember whether there was a deposit when we booked, but if so it was a fairly nominal one.
Yes, *sigh*
And yes... all those miles!
