Shake Shack I don't bother customizing, their burgers and fries are good as is. In-n-out I cannot do without major tweaks. (3 by 2 +both grilled and raw onions + extra toasted bun, fries well-done + grilled onions + spread on the side).

But if you're in SF, then Super Duper is the clear winner

Super duper is definitely one of the best as is Hi-Way. I will not eat anything at shake shack nor in and out. Yesterday we were in San Mateo for track and we fetched shake shack for our son who had finished his competition and was just there to cheer his teammates (as required for all players on the track team). After we brought his food, I asked my wife where she wanted to get luncheon. I also said that we could go to lunch at shake shack if she wanted, but that I would just eat at the nearby Refuge. Thankfully she instead suggested Kajiken.

Shake shack is a better burger with premium price. In n out is a good value

only problem with Super Duper is that it makes you want to sleep after you eat there. Plus its only Bay Area.
