Another upgrade question

When upgrading a transatlantic ticket(with miles), is it origin to destination or just the transatlantic segment?
For example: When I fly TLV-EWR-DTW is the upgrade only for the TLV-EWR segment or for the domestic segment as well?

The upgrade is for the entire cabin where a F/C cabin exists.

Of course ofter the domestic leg is "not available," especially if you go between two hubs, e.g. EWR-CLE. Of course when that's happened to me, I've been on mileage upgrades, which don't seem to have any particular advantage for the domestic upgrades. BF ticket, you should have no troubles.
