International upgrading

I am baffled by CO's international upgrade policy. Specifically, I've read on here about a 30-day buffer zone and only certain fares being upgradable, but have found nothing on continental's website about this 30-day thing or exactly what fares qualify. So can anyone point out a specific place where one can read the official policy?

Most official fares ( no consolidators!) in a fareclass higher than V ( no T ,Q, V)
Most of the time K or B will be allright.
Ugrade seats are usually relased 301 days in advance if any( that is not official policy but experience)
