The FAQ state that the co-pay is taken at time of request, even when on a waitlist.

Going LAX-ARN next month and Continental Gold.
Q1. Do they ever upgrade elite members on international flights like they do domestic flights?
Q2. If I go with the mileage upgrade option what is the deal on an M fare? Seems like 15,000 miles + $250 each way which seems like a bit of a rip-off

Be sure to play with the fare classes and even routings.
When getting a ultra low fare the up fare is higher but the miles are the same unless you get a full fare (Y) ticket.
Sometimes getting Lowest Refundable will cost less as the up fare is lower and still paying the same miles as non Y fare.
But, also try lowest first class fare as I have seen this very close to the Lowest Refundable when adding in the up fare, then you don't use any miles.
Also, you might try booking one leg in first and the other in coach.

You are right, I am not sure where I got 15,000. I might have been mixing that up with another airline/trip

upgrade with orbitz ticket
I recently purchased 2 economy class round trip tickets through orbitz for a flight from ATL to Amsterdam - is it possible to upgrade with cash ? Sorry they are with KLM
