From an Avios.com account, once logged in, from the "My Account" drop down menu, bottom left, under "your Accounts", there is "combine my avios".

From BAEC, when logged into the executive club section, go to "Manage my account", and it is the bottom option in the main bit of the page - "Combine my Avios".
