If it is a fully refundable ticket then the airline will refund all money paid
If there is a cancellation penalty , then this will be applied before getting the refund

If you booked online at aa.com, you should be able to cancel online with AA. Alternatively can telephone

If you booked via a travel agent, it depends on the travel agent whether it can be done online - also check whether the agent charges a service fee

Just tell the agent at the ticket counter that your Grandmother needs assistance, and ask them to issue you a "gate pass". That's usually granted without much questioning, and saves you the hassle of a refund.

Not an excellent idea; though it has been done, it is against AA Conditions of Carriage. If repeated, can be risky.

Should be easier to secure a gate pass to escort your frail elderly, disabled relative or unaccompanied minor to the gate.
