It was the Gold Coast Travel program, which may have had a slightly different name, established in the early 80s (not sure exactly the year). Gold ingots were some of the awards, unfortunately, I didn't quite qualify for the first ingot when the program changed. I have been a member since 1986. I'm sure some of our other FTers can give you more details.

Alaska was giving out those ingots in June of 1979. I know cuz I got one on a SFO-SEA flight aboard one of AS's brand new 727-290s.

As for the Mileage plan, I don't know when it started, but according to my card, I've been a member since June of '84.

Mileage Plan started in June of 1983 under the name "Gold Coast Travel Plan". Perhaps we might see some sort of Mileage Plan 20th anniversary promotion next June.

Jeffrey Pinneo, now CEO and President of Horizon, was supposely "responsible for the original implementation" of the Gold Coast Travel Plan.

The first frequent flyer program was American's AAdvantage program launched in May of 1981. United responded by announcing Mileage Plus a few days later.
