I think you meant AP# not PNR. 
I had similar experiences with bookings this year. I called in to choose seats as a new SE, and each time the agent said it showed an old status. I just mentioned "early qualification" and they re-entered my AP# to fix things.
The safest is to always have the AP# re-entered. But now that you've cleared an e-upgrade, they would need to cancel your upgrade first in order to re-eneter your number, I think. Then re-enter your upgrade.

As an aside, for any other bookings you've made, I would just have your AP# refreshed on all of them.
I made a booking early June as E50k, cleared E75k in July, and made SE in Sept. When I called to pick exit row seats on these June bookings, they were still showing E50k to the SE phone agent (despite hitting E75k in between).

When I refresh my AP#, does that cancel the existing confirmed upgrades? As I wouldn't want to lose those, I have 2 segments so far.
Also, follow-on question: if there's no R space, I cancel my upgrade request and re-apply it - do I go to the bottom of the queue (of SEs, I suppose), or does that get evaluated each time there's R space released - i.e. independent of WHEN you request an upgrade chronologically, your status will be evaluated only on space availability?

I'm guessing if it's not asking me for the $750 add-on then it already sees me as SE, so I probably better just chill out for now.

I've seen that before where I am waitlisted and R goes above 0. Maybe it runs the waitlist process every hour or few hours and you got lucky!
Today my upgrade cleared AFTER web checkin on SFO-YYZ. It made a mess of my record and I couldn't check-in again after they deleted it but ^^^
