Terrible miles earnings on LH flights

You may still get credit for the last flight, though as an H fare. It depends a bit on how Lufthansa handles these upgrades, and if it would be credited automatically or not. A manual retro-credit may be difficult.

Personally I would try to keep your miles in as few programs as possible. If you already have an M&M account you could credit there, but if your main programme is M+B I would stick to that. Miles have much better value if they are in an account with more miles. Or in other words, and M&B account with just a few thousand miles in it is not very valuable at all.

When upgrading an A3 flight with a voucher the ticket is reissued and mileage based on the original booking class are only creditable to M+B.
When using any other program this will not happen automatically.
But as Xandrios said, really depends on your primary program.

This will not happen automatically, as you say, but neither through retrocrediting: if one's using an upgrade coupon, one always flies as a member of M+B. You can't use the coupon without including your M+B number in the booking.

If you have enough time between sectors, you can swap and change for each one (check-in, transfer desk or some lounge agents can do it if you have no means to do it online).

not really. As very often you have choice, I much prefer other options as I do not live in Germany, Austria or Swiss.
