No, there used to be supply of forms (and tables and pens) just before the ICE checks, but they are gone now* to make way for the NEXUS lane (although there's still plenty of room for them AFAICS). And make you sure you have completed the form before you try to join the ICE line - they have a moat dragon who will bar entry* otherwise
