Some of the points you mention above are well and duly noted. They are considered by all the good FAMs I know and they try not to do them. Some, like the preboard, are airline policies that FAMs hate but must abide by for the nonce.
Unfortunately some FAMs don't take their jobs as seriously as others. You have some in every agency. And others who ordinarily would don't because they feel they've already been burned by preboarding so why bother?
