Some of them do have that very high cost to the caller, they have special numbers in Liechtenstein, Estonia, or Iceland that are very high cost.
The one I mentioned two posts above has its incoming numbers on the Isle of Man. Calls to it are billed at the same rate as calls to other UK cell phones. So, while the calls are still more expensive than calls to UK landlines, they are more reasonable than many of the calls to the other international SIMS.

I think the problem with some of these cards is that the cost to the caller to you (the card user) is very high. So incoming calls are free to you but you are sort of shafting whomever is calling you.
