Smallest one I've ever flown into on a commercial flight is San Pedro, Belize. One paved airstrip that comes to an end right smack in the middle of the town. A shack for a "terminal". Exit your plane, walk about 20 steps, and you're on a city street. It was kind of fun flying in there in a Cessna sitting next to the pilot.
