No "upgrade: confirmed" appears if the upgrade clears at booking (or as soon as it's requested).
FWIW, all of my upgrades that confirm at booking still say "upgrade: confirmed" on My Itineraries regardless of whether they were initially waitlisted. I'm looking at a couple right now in that situation, both say "upgrade: confirmed". But still, as long as the seat assignment is correct, I would venture to say that all is OK.
