Bob - Thanks for your thoughts 1 and 2 - defintely things to consider.

#4: We do look for funding anywhere we can, but sometimes it is restricted since these trips are sponsored by our church. The doctors and nurses (and everyone that goes) is volunteering their time, etc. Each person has to raise the money to cover their costs, maybe about $800 per person. (Our church budget also kicks in some money to the overall trip.) We also look for donations of supplies, and we buy the prescriptions and vitamins that we cannot get donated.
