When did you find out about the upgrade? Did you discover this while doing online checkin?
If so, I noticed the airport/online checkin system doesn't name match the elite number to the name for the purposes of assiging "Elite Access" to the boarding pass. Theoretically, if you online or kiosk checkin while R inventory is available, that person will get an upgrade without a name match.
They have an issue where sometimes the R upgrade inventory opens up before the EUA sweep runs. When you checkin online or at the kiosk, it just looks for "R" inventory when asking whether you want to upgrade. If it's available, you get it, since the only trigger for that question is elite status in the FF number.
However, that doesn't mean an EUA will run on CO without a name match.
Never seen an upgrade actually happen. The time I noticed this was an accident where my # got put into both reservations when we booked at CO.com (but separate PNRs). Having your WP# show up twice for the same flights for flight credit on a regular basis is an invitation to losing your status and account.
