Here's my caveat: This is going back several years, so no telling if either program is currently receptive to a request for review. I do recall that at least one of the incidents seemed to be the result of a technical problem on their end, but they never explained what happened, just that I was reinstated.
EDITED TO ADD: I'm a pretty fast reader, and it's my understanding that some of these survey programs will ding you if they think you're blowing through the survey without giving much consideration to your answers, so that might have tripped me up, not sure. I have since made an effort to slow things down a bit when I answer surveys, just to be safe.
Definitely the case lately. I’m sure they have implemented timers on questions. I find I’m screened out after a min or two if I’m answering the first line of questions too fast. I think it’s to do with bots doing surveys. The amount of additional questions under checking your paying attention has quadrupled
