The only other possibility that comes to mind — but it’s a legitimate one — is a displaced business class passenger. If somebody is booked into J and has IRROPS, and the J cabin is full, they can be put into PE or even Y with higher priority than you’d have — but, to your point, you’re liable to die on the upgrade list one way or another.
