Conversely, If the staff were failing to maintain safety regulations, why would you , as a bystander, not inform the company?

That would depend on the airline as to whether allowed. On BA, for example, it would be perfectly permitted and could have been selected by passengers at time of booking. On AA, it shouln't have been permitted
