I cannot modify or create a new reservation.
(1) I accessed Hyatt website to modify my reservation, after it showed the final price page to reflect the new change,
'Sorry, we're unable to update this reservation. Please contact us.'
(2) I set up a new reservation, after I entered my credit card information, it didn't come back any confirmation page.
Is something wrong with the current HYATT system? Or just bad luck?

Sounds like just system issues.
I would double check your upcoming stays on Hyatt.com to see if the new booking went through if not then just book it.
