My TAP issued boarding pass would not work a couple of weeks ago in YYZ. My incoming flight was early, but I am fairly certain by the time I got to the MLL it was inside the 3 hours (not that it should matter on a connection). Same situation as Adam. In my case, I chalked it up to the TAP boarding pass which listed all 3 flights and bar codes on the one paper BP. Maybe the scanner could not differentiate which one to scan.
