When booking on the Mr and Mrs Smith website you can input your gift card code. This will turn the booking into a prepaid booking. If the gift card doesn't cover the whole amount, you need to pay the remaining amount by credit card at the time of booking. You can also use a gift card on an existing booking by either calling Mr and Mrs Smith or using the chat function. Again it will turn the booking into a prepaid booking. I have done this a couple of times already.
Regarding rates being higher on Mr and Mrs Smith I don't find this to be the rule. I have even seen lower rates on the Mr and Mrs Smith site than on the hotel website. Also Mr and Mrs Smith has a Best Rate Guarantee. However, I haven't tried out that process (yet). All the agents at Mr and Mrs Smith I have talked to until now, were extremely polite and helpful. So far I am a big fan.
