I'm no longer sure what I put as a reference, but I think it was my first-night hotel. The key points in the form seem to be don't be a journalist and don't have Pakistan connections.

They'll also want an address from you in the landing form you present at the immigration desk. The days when airlines routinely distributed these onboard seem to be over, so you should pick one up from one of the tables holding them in the run up to the immigration hall. They're pretty short and simple and you can fill them in while waiting in line.
