Well, it's come to this. I faxed copies of my ID, purchase receipt and gift cards on 5 separate occasions but US Bank kept rejecting them for dubious reasons: wrong fax number, not clear enough, not large enough, can't accept reverse image format (?), use camera not scanner, etc. They now tell me to mail it to them.

So when I contact California's AG, should I use the Ohio address you provided? US Bank shows their Minneapolis address on their website. Which phone number should I use? The one on their website (800-USBANKS) or the one on the back of the gift card (888-853-9536)?
