I encountered the same issue with my BOA Alaska credit card. But it went through when I tried the second time with exactly the same info. I would suggest you to give it another try. My 2 cents:
1. Yes if the booking didnt processed then you shouldnt be charged but contact the credit card company if it posted later
2. It is possible but I didnt see any artical mention about it
3. When I encountered the issue, the webpage stayed unchanged at the same payment info page. I would suggest checking booking page or contact CX customer service to pay via phone.
