The 45 minutes at FRA is a hit or miss according to how congested security and immigration will be. Munich might be challenging as well, as it will depend on whether FRA-MUC will depart on time, and you also need to factor in the transfer time to the new terminal (and btw you'll need to clear security also in MUC enroute to the USA).

As a rule of thumb you usually don't, although some times you can randomly (or whatever?) picked for a secondary
security check.
That being said 45 Minutes in Frankfurt in general always seemed very doable for me, but if you if you're not qualified for the automatic border control it could be very tight.
