Once you are at the lounge, there is no direct way to exit to immigration. You'd need staff assistance to get back down to the arrivals level. So if you want to go in to town, you'd basically need to go directly from arriving, and catching up on lounge time after your excursion.

I seem to remember there are lockers in connection with the Keikyu station.
