Yes, the seats will go out empty if no one purchases them or uses other upgrade instruments (such as miles or Global Upgrade Certificates) to upgrade to them. The chart showing Medallion complimentary upgrade eligible seats can be found on this web page --

Only Comfort+ seats are complimentary upgrade eligible on long-haul international flights (with a Delta One cabin) while Premium Select and Delta One seats are not. There's no point in repeatedly checking flights for comp upgrades as it won't happen.
