I’m currently using Allianz. I purchased a plan for a trip in 2020 that was cancelled so I never filed a claim. They contacted me a year later suggesting I apply for a refund. They promptly provided the refund with minimal hassle.

I have two big trips where I’ve purchased a policy with them. I also use their app which allows me to access the policy and assistance quickly.

I’ve noticed that a yearly plan is cheaper then it cost me to buy two trip plans, so I’m going to look at yearly plans from now on.
I don’t buy the insurance through the airline’s process, I just go to Allianz directly.
