It's hit or miss, some airlines will ask you for it and some won't. I was asked by AC this past week and there was someone in another thread somewhere that was forced by UA (I think?) to buy a last minute travel policy at check in. Now when I transferred on to my EK flight at LHR the EK staff asked me for my vaccination certificate again but not the health insurance.
