It was long, but not too bad. The ANA lounge at Narita was open, but it was quite crowded. I managed to take a shower at least.

Yes, I believe having proof of onward travel is a requirement when traveling to Malaysia. I had a separate one-way ticket booked that I showed.

Yes, travel insurance covering covid is a requirement (minimum 20,000 USD coverage). I use SafetyWing Nomad Insurance.
