Not always , it depends on the fare you purchased , because of covid they made new temporary rules , but the key word is temporary

Yes , others moved to more restrictive rules , or some still havent returned money they promised to refund

You need to check the rules of the fares you purchase , there everything is explained , often in complicated terms i agree , but it is there
