From my experience Italian cities do not have a standardised process for their health pass. It varies from region to region. In Naples there is one location where you can show your CDC card and convert it to the Green Pass. I do not know which one but I had two friends in Napoli who definitely had the Green Pass and got it by showing their country's proof of vaccination, one from Mexico, and one from the US.

I was never denied entry to anything by showing my US CDC card, but it does get a beating having to be shown everywhere.
How long ago were you in italy showing your US CDC card? Did you ever show a 'jpg" or phone version of it (rather than paper)? Did it work just the same?
